I've noticed several PWAD MIDI tracks that fade out at the end are staying at zero volume until they get to the fadeout part again (which is the only part that's audible from then on on that map).

DM96.wad seems to be a pretty reliable test case: map01 and map32 are nearly identical tracks, map32 loops fine but map01 has this fadeout bug.
